Chapter 08.01 Business Registrations

Section 08.01.120 Registration – Application

1. The application for a registration shall be made on forms which set forth the:

1. 1. name of the applicant,

1. 2. applicant's residence,

1. 3. place of business,

1. 4. nature of the business,

1. 5. number of personnel, and

1. 6. amount of the registration fee.

2. Applicants whose circumstances fall under the definition of SMC 10.40.010 must further disclose information as required in SMC 10.40.030.
