Section 10.41A.050 Application

The applicant is required to set forth in the application:

  1. the name and address of the applicant;
     
  2. the address or other description of the location(s) where the subject will be employed;
     
  3. the full name, date of birth and address of the subject;
     
  4. all facts known to the applicant regarding the subject’s qualifications under SMC 10.41A.060;
     
  5. the reason why the applicant believes the subject should be commissioned; and
     
  6. a description of the proposed responsibilities and functions of the subject and of the subject’s present scope of employment.
