As part of restructuring the Continuum of Care (CoC) Board, many of the subpopulation subcommittees are working on system gaps analyses of their respective service tracks within our continuum of care of services. Utilizing the Support Services for Veteran Families (SSVF) System Assessment and Improvement Toolkit released earlier this year, the Veterans Leadership Team and Veterans CoC Sub-Committee will evaluate the current track for veterans experiencing homelessness in Spokane, starting with all points for entry into the system and going all the way through to permanent housing. They will also review prevention services as well as consult with clients that have moved through the continuum of services to gain their insight into any barriers and opportunities for improvement.
Identify the current system's key stakeholders, capacity, components, processes, participant flow, and data collection to create a collective understanding.
Assess the current system's programming, components, and participant flow to gain a community-wide understanding of how the system is currently designed.
Use the findings from the assessment as a way to envision the community's desired system response.
Develop an action plan to set concrete steps to achieve the desired outcomes.
