The Spokane HMIS Team assists over 200 users with their work to end homelessness in the City of Spokane. This page is designed to complement that work. Here you will find tools to help users navigate and increase the functionality of ClientTrack. You will also be kept apprised of upcoming reporting deadlines and the latest news that could impact their HMIS experience.
These templates were revised October 2016, and may be revised in the future. We understand that you may have a need to customize and edit these forms and please feel free to do so. However, all forms that are used in place of direct entry into the HMIS need to be approved by the City of Spokane HMIS staff before implementation so that we can ensure each meets all data collection requirements. The City of Spokane reserves the right to request changes and make the final determination of the use of all data collection forms, custom or baseline.
